Ready Steady Rehab accepts referrals for older adults and people living with disability across Melbourne. Referrals may be made by clients, families, support coordinators, care managers, health professionals and referral partners.

• 2. We discuss the next steps. Our team will review the information and make contact.
• 3. Home assessment. A physiotherapist visits the client at home to begin personalised care.
